I've seen people do both. It really depends on how you feel about her. If you truly feel like she is your best friend, then put her before everyone else. Otherwise, be honest and put your friends and family first. She should understand...if you've known your friends and family longer than you've known her, it makes sense that they would come first. Really though, I think the "Top Friends" thing is a little ridiculous, and so I removed it from my profile. I didn't want to deal with ranking people. Each person in my life is a "Top Friend" in their own special context. I have my school friends, my work friends, my neighborhood friends, my relative friends...it is not a matter of one person is head and shoulders above everyone else.
